Participation in the Broker Endorsement Program with Enhanced Fees is established when a Broker places our Link, as designed by USFSB, on the Home Page or other mutually agreeable prominent location of the Broker’s official web site with the acknowledgment that USFSB is the Preferred Source of Group Health Insurance. This Link will open a search page for all of USFSB’s available Association Health Insurance Plans. This search page will require that the visitor enter certain contact information and will not allow the visitor to migrate from the Broker’s web site. The Broker can elect to limit the search to USFSB’s Association Group Sole Proprietor Health Insurance Plans; however, the Broker will still receive the enhanced fees on all qualified sales and referrals enrolled during this participation.

Once a search is initiated, an e-mail will be sent, simultaneously, to both USFSB and the Broker with the visitor’s contact information. As soon as possible, after receipt of the e-mail, USFSB will contact the Broker to determine whether or not the Broker elects to pursue the lead directly or have USFSB pursue the lead as a Referral. Brokers may elect not to participate in this e-mail process.

In any case, Brokers will be able to log into the USFSB Broker Section and obtain both a count of the number of “hits” to the Link and a list of all visitors who filled out the contact information to start a search.

Participating Brokers will receive $25 per month per enrollee plus $30 from the yearly membership dues for each qualified business Broker signs up as a new Member of USFSB and the Broker enrolls one or more owners/employees in any one of USFSB’s association health insurance plans. These fees and dues will continue to be paid as long as the business is an active dues paying member of USFSB and is and remains current with their health insurance premiums and only while Broker and USFSB are participating in this program.

Participating Brokers will receive $15 per month per enrollee plus $30 from the yearly membership dues for each qualified business referral that joins USFSB as a new Member and enrolls one or more owners/employees in one of USFSB’s association health insurance plans. These fees and dues will continue to be paid as long as the business is an active dues paying member and is and remains current with their health insurance premiums and only while Broker and USFSB are participating in the program. A qualified referral is one which results in a new membership and enrollment of one or more owners/employees in one of USFSB’s association health insurance plans within two months of the referral. A referral will also not qualify if the referred business is an existing member of USFSB or an existing prospect of USFSB or for any enrollee who is an owner/employee of an existing USFSB member business or existing USFSB prospect. In this case, there will be no enhanced fees or the sharing of dues or other fees that may be collected.

These enhanced fees for sales and qualified referrals will be paid whether or not the enrollment is as a direct result of the Broker Endorsement Program as long as all other terms of this program have been met and only while Broker and USFSB are participating in the program.

Either the Broker or USFSB may discontinue the Broker’s participation upon thirty days written notice or sooner if the Link is removed or disabled. In that event, the Broker will continue to be paid the enhanced fees for the new memberships and enrollments made during the period of the Broker’s participation, as long as the business is an active dues paying member and is and remains current with their health insurance premiums. All sales and referrals, thereafter, will be paid based on USFSB’s standard fee schedules.

This Broker Endorsement Program is limited to USFSB sponsored association health insurance plans for which members are billed directly by USFSB. The Insurance Guidelines, Insurance Rules, and Insurance Disclaimers as posted on USFSB’s Web Site will also apply.
