Once you enroll in the program, your Medical Discount Card can be used, at participating health care providers, to obtain a reduced cost for the services that are rendered as long as you are prepared to pay for the services at that time.

The Medical Discount Card Program can provide you with savings of up to:

• 10% - 30% on Doctor Bills
• 10% - 30% on x-rays and lab work
• 10% - 30% on Diagnostic Services
• 10% - 50% on Prescription Drugs
• 20% - 60% on Eye Care
• 20% - 50% on Dental Bills
• 10% - 30% on Hospital Bills

The Medical Discount Card Program has the following Features and Benefits:

• No Social Security Number required
• Easy to use
• No Paperwork
• No Health Restrictions
• Multilingual Customer Service
• Point of Service Savings

All of these savings and benefits for only $60.00/Month for Individuals or Families. There is a one time Set-Up Fee of $10.00. Family includes your spouse and all legal dependents.

3. This discount program may not be used in conjunction with any other discount plan or program.
4. Savings are based upon the Provider’s normal fees. Actual savings will vary depending upon the location and specific services or products purchased.
5. This Program makes no warranties express or implied concerning services or care provided.
6. This Program does not make payments directly to the Provider of medical services.
7. You are obligated to pay for all health care services and products but will receive a discount from those health care providers who have contracted with American Health Services.

Cancellation Policy:
If you cancel for any reason within 30 days, you will receive a full refund. The Set-Up Fee is non-refundable.
