Proud Member Referral Program
Participation in the Proud Member Referral Program is established when a USFSB Member places our Proud Member Link, as designed by USFSB, on the Home Page or other mutually agreeable prominent location of their official web site and will continue until the participant ceases to be a member of USFSB; the Link is removed or disabled; or either party terminates the participation upon thirty days written notice to the other party. This Link will open a search page for all of USFSB’s available Association Health Insurance Plans. This search page will require that the visitor enter certain contact information in order to initiate a search.
Once a search is initiated, an e-mail will be sent, simultaneously, to both USFSB and the Participating Member with the visitor’s contact information.
Participating Members will receive $10 per month for all qualified referrals who enroll in one of USFSB’s association health insurance plans, as long as the business is an active dues paying member, except as it may apply to Organization referrals, and is and remains current with their health insurance premiums. Participating Members will also be entitled to be part of USFSB’s Affiliate Program.
A qualified referral is one which results in an enrollment of an association health insurance product within two months of the referral. A referral will not qualify if the subject business is an existing member of USFSB or an existing prospect of USFSB. In this case, USFSB will notify the Member, as soon as possible, that the referral does not qualify for this Program. There will be no sharing of dues or other fees that may be collected.
This Referral Program does not apply to any referral for which a broker is involved or in which USFSB is or becomes obligated to pay a fee to any broker.
The subject Association Health Insurance Plan must be maintained and current in the payment of premiums and fees for, at least, three months before the monthly referral fee begins to be paid, at which time, they will be paid retroactively to the first month of enrollment.
For Participating Organization Members, as soon as possible, after receipt of the e-mail, as stated above, USFSB will contact the Organization’s designated representative to determine whether or not the Referral is an existing member of the Organization. If the referral is an existing member of the Organization, the referral need not separately become a member of USFSB, upon enrollment in an Association Health Insurance Plan, for as long as the referral is a member, in good standing, of the Organization and the Organization maintains it’s membership with USFSB. If the referral is not an existing member of the Organization, the referral will need to become either a member of the Organization or a dues paying member of USFSB at or prior to the time of enrollment.
This Program is limited to USFSB sponsored association health insurance plans for which members are billed directly by USFSB. The Organization Guidelines, Insurance Guidelines, Insurance Rules, and Insurance Disclaimers as posted on USFSB’s Web Site will also apply.
