USFSB is pleased to announce that, through a special arrangement with Rivlin Rx, all of our members with or without prescription coverage can receive a USFSB prescription drug discount card. This exciting value-added program is designed to provide prescription drug savings to our members at a very low cost.

The USFSB Prescription discount program is more than a discount card. This program offers the member the opportunity to receive their prescriptions from their local retail pharmacy, a mail order pharmacy, or even a Canadian pharmacy. Here are a few examples of the prices for some prescription medications using the USFSB program:

Upon receiving your application for this program, you will receive your Liberty Care Rx ID cards and welcome letter.

There is a one-time enrollment for Rivlin Rx of $20 per individual employee or $32 per employee and family and only $36 per employee, for six months, which will be billed by USFSB. (If coverage is terminated for non-payment, there is a $10 re-instatement fee in addition to the payment due.) There is no age limit for the dependents and the employee can even enroll their parents living with them as dependents.

Once your company is enrolled you will receive personalized enrollment forms for new employees and yearly status change forms for your review and action.

This program will generally pay for itself with savings from the first prescription. Employees that use this card monthly save much more than the cost of this program.

The card can be used by everyone enrolled living in the same household, and can be used immediately. Savings range from 10% - 60%, and are applied at the time of purchase. USFSB cardholders may visit any of the thousands of participating pharmacies. There are no claim forms or paperwork that needs to be filed. Simply present your USFSB card at any of the 53,000 participating pharmacies nationwide to begin saving.
